Step 1:

Gather these items:
- Copy of a current utility bill for residency verification
- Copy of parent/guardian driver's license or ID
- Copy of your child's birth certificate
- Copy of IEP & ETR
OR - Copy of AEP and Clinical Autism Diagnosis Report
Step 3:
Fill out this paperwork:
